Kokichi controls Mechamaru and the other puppets while remaining in a room. Even though on the outside, Mechamaru looks quite cheerful and interacts a lot with other sorcerers, his real body is in a lot of pain with bandages covering his whole body. Even Kokichi no longer has his right arm because it was severed, and his two legs are rendered useless and can't be used to stand up anymore.

The Reason Why Mechamaru Betrayed His Fellow Jujutsu Sorcerers
You might feel sorry for Mechamaru who is always in pain, so you wonder what benefit he got from betraying the Jujutsu sorcerers.
